How to Qualify

According to Ohio Low House Bill 523, patients who have one or more of these 28 medical conditions qualify for the possession of medical marijuana (cannabis). Flower, edibles, capsules, tinctures, oils, and topical forms are available in Ohio. 

Qualifying Medical Conditions under Ohio Law: 

  • Acquired Immune Deficiency Syndrome (AIDS)

  • Alzheimer’s Disease

  • Amyotrophic Lateral Sclerosis (ALS)

  • Arthritis

  • Cachexia

  • Cancer

  • Chronic Migraines

  • Chronic Pain

  • Chronic Traumatic Encephalopathy (CTE)

  • Complex Regional Pain Syndrome (CRPS)

  • Epilepsy

  • Fibromyalgia

  • Glaucoma

  • Hepatitis C

  • Huntington's Disease

  • Inflammatory Bowel Disease

    • Crohn’s Disease

    • Ulcerative Colitis

  • Multiple Sclerosis

  • Parkinson’s Disease

  • Positive status for HIV

  • Post-Traumatic Stress Disorder

(PTSD must be diagnosed by Psychiatrist)

  • Sickle Cell Anemia

  • Spinal cord disease or injury

  • Spasticity

  • Terminal Illness

  • Tourette’s Syndrome

  • Traumatic Brain Injury (TBI)

  • Any other disease or condition added by the state medical board

MMJ's New Patient Process

We have an easy process to become qualified for medical marijuana. 

Schedule your first consultation by calling us at (440) 510-8470 or by completing the Appointment Inquiry Form at the bottom of this page. Please allow up to one business day for a response.

Bring these documents to your first consultation: 

  • State of Ohio Photo ID

  • 1 medical record from a physician from the past 12 months with one of the qualifying medical conditions​​​ (qualifying conditions listed above)

Fill out patient forms in the waiting area or for your convenience, the PDF forms located below prior to your appointment.

Pay $250 first time fee or schedule an interest-free payment plan.

Have an in-person or virtual medical evaluation with Dr. Sidari. 

The office registers you with the Ohio Medical Marijuana Board.​

Complete your Ohio Medical Marijuana Registration, by paying a $50 state fee, or $25 state fee if you are a veteran or SSI/SSD (documentation required).
